Terms of Use for Personal Information

Chapter 1: General Provisions

Article 1 (Purpose)
These Terms are intended to prescribe the conditions and procedures for the use of services provided by JAYJUN Plastic Surgery Clinic (the “Clinic”), as well as other necessary matters, including the rights, obligations, and responsibilities of the Clinic and its users.

Article 2 (Effect and Amendment of Terms)
(1) These Terms shall become effective upon being publicly disclosed to users.
(2) The Clinic may amend these Terms in the event of changes in circumstances or for significant business reasons, and any amended Terms shall become effective in the same manner as set forth in the preceding paragraph.

Article 3 (Supplementary Rules)
Matters not specified in these Terms shall be governed by applicable laws and regulations.

Chapter 2: Membership Registration and Use of Services

Article 1 (Definition of Member)
A “Member” refers to an individual who is deemed eligible by the Clinic, agrees to these Terms, completes the membership registration form, and is issued an ID and password.

Article 2 (Formation of Service Agreement)
(1) A service agreement shall be formed upon the Clinic’s acceptance of a user’s application and the user’s consent to these Terms.
(2) Any individual wishing to register as a Member and use the services must provide the personal information requested by the Clinic.
(3) Upon approval of the application, the Clinic shall notify the user of the Member ID and any other information deemed necessary by the Clinic.
(4) The ID entered at the time of registration may not be changed, and only one ID shall be issued per individual.
(5) The Clinic shall not approve applications falling under any of the following:
a. Where the application is made using another person’s name
b. Where the application is not made under the applicant’s real name
c. Where false information is provided in the application form
d. Where the application is made for the purpose of undermining public order or morals

Article 3 (Use and Restriction of Services)
(1) In principle, services shall be available 24 hours a day, year-round, unless there are special operational or technical difficulties.
(2) The service hours under the preceding paragraph may be restricted, with prior notice to Members, where necessary for system maintenance or other reasons determined by the Clinic.
(3) Online consultation services may not be available on a 24-hour basis depending on the personal circumstances of the consulting physicians.

Article 4 (Service Fees)
(1) The services shall be provided free of charge to all registered Members.
(2) In the event that the Clinic converts the services to a paid service, the timing, policy, and fees shall be announced in advance through the service prior to implementation.

Chapter 3: Termination, Re-registration, and Restriction of Use

Article 1 (Termination of Service)
(1) If a Member wishes to terminate their membership, the Member must personally request termination by sending an email to the administrator.
(2) Upon submission of a termination request, the Member shall provide identifying information, including name, ID, telephone number, and reason for termination. The Clinic shall verify such information against the registration records before processing the termination.
(3) Termination shall be deemed completed when login using the existing ID and password is no longer possible.

Article 2 (Re-registration)
(1) A user who has terminated their membership pursuant to Article 1 may request re-registration by sending an email to the administrator.
(2) Upon requesting re-registration, the user shall provide identifying information, including name, ID, and telephone number, and the re-registration shall be processed accordingly.
(3) Re-registration shall be deemed completed when login using the existing ID and password becomes possible.

Article 3 (Restriction of Use)
The Clinic may terminate the service agreement or suspend the use of services for a specified period without prior notice if a Member engages in any of the following acts:
a. Acts contrary to public order and morals
b. Acts related to criminal activities
c. Planning or executing the use of services for the purpose of harming national interest or public interest
d. Unauthorized use of another person’s ID or password
e. Damaging the reputation of others or causing disadvantage to others
f. Registering multiple IDs under the same user
g. Disrupting the proper use of the service
h. Violating applicable laws or the conditions of use established by the Clinic

Chapter 4: Limitation of Liability

Article 1 (Online Consultation)
(1) The Clinic shall make its best efforts to maintain the security of consultation content so that it is not disclosed to third parties other than the consulting physician and service administrators. However, the Clinic shall not be liable for disclosure or loss of consultation content in the following cases:
a. Where the user’s password is disclosed due to the user’s negligence
b. Where the user deletes the consultation using the “delete” function
c. Where disclosure or loss occurs due to force majeure or circumstances beyond the control of the Clinic
(2) For the purpose of providing comprehensive and appropriate responses to consultations, attending physicians and specialists in relevant departments may refer to the consultation content and responses.
(3) Consultation content conducted through the service may be used for the following purposes after removing personally identifiable information:
a. Academic activities
b. Publications such as printed materials and CD-ROMs
c. Portions of service content such as FAQs and recommended consultations
(4) Responses to consultations are based on the individual medical judgment of each physician and do not represent the official opinions of the Clinic.
(5) The Clinic may refuse to provide all or part of online consultation services in the following cases:
a. Repeated submission of the same consultation
b. Use of expressions that are inconsistent with common standards
c. Requests for a diagnosis
d. Requests regarding treatment costs, examination fees, or pharmaceutical prices

Article 2 (Information Services)
(1) The information provided through the service is general and for informational purposes only, and shall not under any circumstances substitute for medical diagnosis. The information or consultations provided are not intended to replace medical diagnosis, treatment, or care. If a Member has any questions or concerns regarding their health condition, they must seek diagnosis from a qualified medical professional. Under no circumstances should a user disregard medical advice or delay diagnosis or treatment based on information provided through the service.
(2) The Clinic does not recommend any specific tests, products, or treatments mentioned in the service. All opinions expressed are those of the individual consulting physicians. The Clinic assumes no responsibility for any documents or consultation content provided through the service.
(3) Acceptance of information provided through the service, including opinions of participating physicians, other Members, or visitors, is entirely at the user’s discretion. Accordingly, the Clinic shall not be liable for any damages, injuries, or other disadvantages arising from the use of any products, information, ideas, or instructions provided to Members.

Chapter 5: Obligations

Article 1 (Obligations of the Clinic)
(1) The Clinic shall make the service available to Members unless there are unavoidable circumstances.
(2) The Clinic has an obligation to provide continuous and stable services in accordance with these Terms.
(3) The Clinic shall process opinions or complaints raised by Members through appropriate procedures, and where processing requires a certain period of time, the Clinic shall notify the Member of the reason and the expected schedule.

Article 2 (Obligations Regarding Member Information Security)
(1) Members bear full responsibility for the management of their ID and password.
(2) Members agree to receive service-related emails sent as part of the service.
(3) In the event of unauthorized use of their ID, Members must immediately notify the Clinic.
(4) The Clinic shall not sell or provide personally identifiable information to third parties unrelated to the Clinic without prior consent of the Member or user. However, the Clinic may utilize voluntarily provided registered information in the following cases:
a. Providing information to developers necessary for developing useful new features, information, or services for Members
b. Providing statistical information about Members and user groups to advertisers (in a manner that does not identify individual Members)
c. Using such information within the Clinic to provide advertisements or services tailored to Member and user preferences
(5) Personally identifiable information (such as name, ID, email address, etc.) may be voluntarily disclosed in communication spaces such as bulletin boards (hereinafter referred to as “Communication Spaces”). In such cases, the disclosed information may be collected, linked, and used by third parties, and users may receive unsolicited messages from third parties. Such actions by third parties are beyond the control of the Clinic, and therefore the Clinic makes no guarantees regarding the possibility of Member information being discovered through means beyond its control.
(6) The Clinic may use cookie technology to enhance the convenience of service use. A cookie is a small text file sent by the server operating the website to the user’s browser and stored on the user’s computer. Cookies help identify returning users and facilitate continued access and personalized services. Cookies do not intrude into the user’s system or pose any risk to files. The Clinic cannot prevent advertisers or related third parties from using cookies. Users may control whether to accept cookies through their web browser settings; however, refusal of cookies may result in difficulties in using certain services, particularly personalized services.
(7) The Clinic may share and utilize Member information between the service and affiliated business partners.

Chapter 6: Dispute Resolution

(1) Any disputes arising in connection with the use of the service, other than those specified in these Terms, shall be resolved through mutual agreement between the parties whenever possible.
(2) In the event that a lawsuit is filed in relation to a dispute arising from the use of the service, the court having jurisdiction over the location of the Clinic shall have exclusive jurisdiction.

Supplementary Provision
These Terms shall take effect on October 26, 2010.



Privacy Policy


JAYJUN Plastic Surgery Clinic (hereinafter referred to as the “Clinic”) values users’ personal information and hereby informs that the personal information processed is limited to the minimum necessary for the purposes of such processing. The Clinic complies with applicable laws and regulations related to personal information protection, including the Act on Promotion of Information and Communications Network Utilization and Information Protection, the Personal Information Protection Act, and the personal information protection regulations established by the Korea Communications Commission. In accordance with such laws, the Clinic has established this Privacy Policy and makes every effort to protect users’ rights and interests. Through this Privacy Policy, the Clinic informs users of the purposes and methods by which personal information is used and the measures taken to protect such information.
In addition, in accordance with applicable laws and regulations, the Clinic respects the rights of data subjects, including the right to access, correct, delete, and request suspension of processing of personal information held by the Clinic. This Policy shall take effect on January 15, 2015.

The Clinic provides prior notice of the scope and purpose of personal information collection at the time of membership registration or in the Terms of Use in accordance with relevant laws and regulations. The items of personal information collected are as follows:

• Items collected: Name, ID, password, email, gender, date of birth, home address, mobile phone number, telephone number, access logs, cookies, IP address
• Collection methods: Website (membership registration, online reservations, online consultations, post-surgery consultations)
※ During the course of service use or service processing, the following information may be automatically generated and collected:
• Service usage records, access logs, cookies, IP address
■ Personal Information Collected During Medical Services
• Required items: Name, gender, date of birth, contact information (including guardian contact information), address, height, weight, blood type, previous surgery history, alien registration number, resident registration number, medical records
• Health information: Medical history, family history, and other personal health information deemed necessary by medical staff for the provision of medical services
■ Personal Information Collected During Payment
• Credit card payment: Card company name, card number, and other payment authorization information
※ If personal information is collected for a specific short-term purpose, such collection will be separately announced.
■ Methods of Collecting Personal Information
• Website, written forms, fax, telephone, consultation boards, email, etc.
■ Security Measures
• Encryption applied during transmission of collected personal information

Article 2 (Purpose of Collection and Use of Personal Information)

The Clinic uses collected personal information for the following purposes. All information provided by users will not be used for purposes other than those listed below, and prior consent will be obtained if the purpose of use changes.
• Identity verification for medical treatment, examinations, and reservation inquiries
• Provision of diagnostic and treatment services
• Administrative services such as billing, payment, and refunds
• Issuance and delivery of bills, statements, certificates, medications, and results
• Outsourced online/offline testing and external test requests
• Communication channels for handling complaints and grievances
• Legal and administrative measures for quality control and clinic operations
• Minimum data for education and research purposes
• Provision of medical information, academic information, and clinic-related information

Article 3 (Provision and Sharing of Personal Information with Third Parties)

The Clinic does not use or provide personal information beyond the scope stated in the “Purpose of Collection and Use” without the user’s consent, except where required by law.
However, exceptions apply in the following cases:
• Identity verification for medical services
• Submission of medical records for insurance claims to the Health Insurance Review & Assessment Service under the National Health Insurance Act
• Where users have given prior consent
• Where required by law or requested by investigative authorities in accordance with legal procedures
• Where provided in a form that does not identify individuals for statistical or academic research purposes

Article 5 (Retention and Use Period of Personal Information)

The Clinic shall destroy personal information without delay once the purpose of collection or provision has been achieved.
• Membership information: Upon withdrawal or termination
• Survey/event information: Upon completion
• Medical records: Retained in accordance with Article 15 of the Enforcement Rule of the Medical Service Act
(Patient registry: 5 years / Medical records: 10 years)
(Retention items: name, address, resident registration number, medical information)
• Consumer complaints/dispute records: 3 years (Act on the Consumer Protection in Electronic Commerce)
• Credit information records: 3 years (Act on the Use and Protection of Credit Information)
• Identity verification records: 6 months (Act on Promotion of Information and Communications Network Utilization and Information Protection)
• Website visit records: 3 months (Protection of Communications Secrets Act)
※ However, personal information may be retained if required by applicable laws such as the Commercial Act.

Article 6 (Destruction Procedures and Methods)

Personal information is destroyed without delay once its purpose is fulfilled.
■ Procedure
Information entered for membership registration, etc., is transferred to a separate database (or stored in a separate physical file cabinet) after the purpose is achieved, and retained for a certain period in accordance with internal policies and applicable laws before being destroyed. Personal information transferred to a separate database is not used for any purpose other than retention unless required by law.

• Electronic files: Permanently deleted using technical methods that prevent recovery
• Printed materials: Shredded or incinerated

The Clinic responds promptly and in good faith to requests from users to access, correct, or delete personal information.
To protect personal information, requests for access, correction, or deletion via telephone, mail, or fax are not accepted; requests must be made through in-person visits.

Users may visit the Clinic to request access, and the Clinic will respond promptly.
■ Correction and Deletion
1. If a user requests correction or deletion and it is determined necessary, the Clinic will process it without delay and may request supporting documents.
2. Identity verification will be conducted using official identification such as a resident registration card, passport, or driver’s license.
3. If there is a legitimate reason to refuse, the Clinic will notify the user and explain the reason.
4. Legal guardians of children under 14 may exercise these rights with proof of relationship and identification.

Users may refuse cookies through browser settings, but some services may be limited.
