Why does the effectiveness of the same facelift surgery vary?  

The cause can be found in differences in the skin, fat layer, and SMAS (Superficial Musculo-Aponeurotic System) layer of the face.

if the skin is thick or tough, or if there is a thick fat layer, the lifting effect may be limited.
